Summer Services Request

Students who were enrolled in Spring classes and are enrolled for Fall classes and want to extend their coverage for the summer may pay a one-time bridge charge of $25 to receive an initial assessment for services or to continue established care. If you are only enrolled in one summer session, then the charge will be $12.50 instead of $25. If you would like to enroll for services, please fill out the following form and a support staff member will assist you with this request.
